Job summary

Northshore Home Health Care is seeking a licensed Physical Therapist to assess referred patients, develop and implement individualized treatment plans, and deliver skilled physical therapy as prescribed. You will work with a multidisciplinary team to ensure evidence-based rehabilitation and continuous improvement in patient outcomes.

The role includes developing care plans, documenting progress, and adjusting treatments to meet goals.

Qualifications

• Doctorate or Bachelor's degree in Physical Therapy.
• Pass the NPTE exam.
• Possess current state licensure or be in process.
• Have at least one year of experience.
• Hold current CPR certification.
