Job summary

Affinity Home Health Care Inc. is seeking a Travel Physical Therapist (PT) to evaluate, diagnose, and treat patients with mobility impairments to improve function and quality of life.

  • Collaborate with healthcare professionals to ensure coordinated care and optimal results.
  • Educate patients and caregivers on exercise, injury prevention, and self-management strategies.
  • Ensure safety by adhering to infection control and safety protocols.
  • Participate in quality assurance and professional development activities.
  • Assist in discharge planning for smooth patient transitions.

Job description
