Fairview Hospital is seeking a Travel SNF Physical Therapist to provide comprehensive therapy services in skilled nursing facilities nationwide. You will evaluate patients, design and implement individualized treatment plans, and collaborate with interdisciplinary teams to promote recovery and maximize function.

The role requires documenting progress, maintaining safety standards, and tailoring therapies to each patient’s needs.

Qualifications

• DPT/DPT-equivalent from an accredited program per APTA.
• Active state PT license required.
• BLS certification may be necessary.
• Strong communication skills and patient-care commitment.

Role:

Travel Skilled Nursing Facility (SNF) Physical Therapist

Responsibilities:

Assist patients in recovering from injuries and managing chronic conditions, providing therapy services in a SNF setting. Ensure patient progress, document treatment plans, and collaborate with healthcare teams to deliver quality care.

Qualifications & Skills:

Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited institution approved by APTA. Active State License required; BLS certification may be necessary. Strong communication skills and a commitment to patient care are essential.

Key Duties:

Provide therapy services, maintain accurate documentation, adhere to safety standards, and support patient recovery goals. Work scheduled days Tuesday through Saturday, with 40 hours per week and 8-hour shifts.
