Job summary

Apex Home Health, Hospice & Home Care is seeking a Travel Physical Therapist to deliver patient care in Skilled Nursing Facility settings across the region. You will travel between facilities to support rehabilitation plans and improve patient outcomes.

The role requires a PT degree, an active state license, and BLS in some cases, with a schedule of 40 hours per week and 8-hour shifts. Collaborative care and accurate documentation are essential.
