Proforma Health is seeking a travel Physical Therapist (PT) who evaluates, diagnoses, and treats patients to improve mobility, reduce pain, and enhance quality of life.

You will develop personalized treatment plans, implement therapy techniques, operate equipment, document outcomes, and collaborate with medical teams, while educating patients and caregivers and supporting discharge planning across diverse locations.

Qualifications

• Valid state licensure to practice as a physical therapist.
