Job summary

CONSUMER DIRECTED SERVICES INC. seeks a Travel Home Health Physical Therapist to support patient recovery and chronic condition care across varied home settings.

Responsibilities include conducting home visits, occasional facility floats, and participation in office meetings while maintaining safety and high-quality care. Must have PT degree, active license, CPR, and willingness to travel nationwide.

Qualifications

• Physical Therapy degree from an accredited program.
• Active State License in the applicable state.
• CPR certification (and possibly BLS).
• Willingness to travel to patient locations and facilities.
• Ability to work independently and manage time effectively.

Responsibilities

• Perform home visits to deliver PT services to patients in their homes.
• Attend periodic office meetings and potentially float to other facilities as needed.
• Follow safety protocols, document care, and coordinate with supervisors.
