Job summary

Consumer Directed Services Inc. is seeking a Travel Home Health Physical Therapist to assess and treat patients in diverse settings within the assigned coverage area. The role requires full-time availability, a valid PT license (AL or multi-state), BLS/CPR, and a driver''s license.

Mileage reimbursements accompany daily patient visits. Typical hours are 8:00 AM to 4:30 PM, Monday through Friday, with occasional weekends and travel between visits.

Qualifications

• Must hold a Physical Therapist license (AL or multistate).
• Degrees: Bachelor''s, Master''s or Doctoral in Physical Therapy from an accredited program.
• BLS and CPR certifications required; valid driver''s license.

Responsibilities

• Deliver therapy services Monday through Friday, 8:00 AM–4:30 PM.
• Travel between patient visits with mileage reimbursements.
• Maintain professional attire (navy scrubs, closed-toe shoes).
• Adhere to safety protocols and maintain documentation.

Role: Travel Home Health Physical Therapist

Assess and treat patients recovering from injuries or managing chronic conditions, providing therapy services in various settings within the assigned coverage area. Maintain documentation, adhere to safety protocols, and ensure quality patient care. Occasional weekend work and travel between visits are required.

Qualifications & Skills

Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited institution, active state license (AL or multistate), BLS certification, CPR certification, valid driver’s license, and ability to work full-time hours. Experience in home health therapy is preferred.

Key Duties
