Job summary

Affinity Home Health Care Inc. is seeking a Physical Therapist to assess patients requiring rehabilitation services, develop individualized treatment plans, and provide skilled physical therapy interventions in accordance with physician orders.

The PT will conduct evaluations, set goals, document progress, educate patients and families, collaborate with multidisciplinary teams, and ensure treatments are aligned with patient capacity and safety.

Qualifications

• Must hold a Doctorate or Bachelor's degree in Physical Therapy with NPTE pass.
• Graduate from an accredited school.
• Current state licensure or in the process, along with CPR certification.

Responsibilities

• Develop and execute treatment plans, provide interventions, assess and modify care as needed, and document patient outcomes to facilitate recovery and functional improvements.
• Conduct patient evaluations, set realistic goals, document patient progress, educate patients and families, and collaborate with multidisciplinary teams to plan and evaluate care.
