Job summary

Sweetwater Care in Butte, MT is seeking a licensed physical therapist to consult with patients, review medical histories, and develop individualized treatment plans to relieve pain and improve mobility.

You will supervise assistants, maintain accurate records, follow evidence-based practice, and collaborate with the health team to ensure high-quality discharge planning.

Qualifications

  • Active state licensure is required.
  • Experience with patient assessment and documentation is preferred.

Responsibilities

  • Consult with patients to learn about their physical condition and symptoms. Review their medical history and referrals.
  • Diagnose movement dysfunction by observing patients as they move.
  • Develop individualized treatment plans.
  • Coach patients and teach therapeutic massage to ease pain and promote healing.
  • Maintain patient records and track goals.
  • Use standardized assessments and evidence-based practice to support interventions.
  • Assess progress and adjust treatments in the clinical record per regulatory requirements.
  • Evaluate treatment outcomes and modify services accordingly, referring as needed.
  • Provide counseling to patients, families, and other providers related to physical disorders.
  • Communicate progress, barriers, and plans with supervisor and health team members.
  • Provide comprehensive discharge summaries per regulatory requirements.
  • Document all regulatory information in the electronic system and maintain patient records.
  • Participate in trainings, in-services, and conferences.
  • Able to supervise assistants and students.
  • Adhere to company policies and code of conduct.
  • Maintain active state licensure.

Job description

Principal Responsibilities
  • Consult with patients to learn about their physical condition and symptoms. Review their medical history and referrals from doctors or surgeons.
  • Diagnose movement dysfunction by listening to patients and observing them as they move.
  • Develop individualized treatment plan
  • Coach patient and teach them therapeutic massage to ease patients’ pain and provide stimulation to promote healing
  • Maintain patient record by keeping track of goals
  • Utilize standardized assessments and evidence-based practice to support clinical interventions.
  • Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
  • Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
  • Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
  • Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
  • Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
  • Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy. Accurately records treatment minutes for services provided per all regulatory and clinical requirements.
  • Participate in facility and company required trainings, in-services and conferences.
  • Able to supervise and support physical therapist assistants; rehabilitation aides and/or students.
  • Adherence to Company required policy and procedures and code of conduct.
  • Active State Licensure
