Job summary

Sweetwater Care in Deer Lodge, MT is seeking a licensed Physical Therapist Assistant to join our clinical team. You will assess movement dysfunction, develop individualized plans, and coach patients on therapeutic massage techniques to ease pain and promote healing.

This role requires active state licensure, strong team communication, and careful documentation of progress and discharge summaries. You will collaborate with physicians, therapists, and caregivers and participate in required

Qualifications

  • Active State Licensure as a Physical Therapist Assistant.
  • Strong communication and teaching skills.
  • Ability to review medical histories and referrals.

Responsibilities

  • Consult with patients to learn about their condition and review medical history.
  • Diagnose movement dysfunction by observing patients during movement.
  • Develop individualized treatment plans.
  • Supervise and support PT assistants and rehabilitation aides and/or students.
  • Maintain patient records and discharge summaries with goals and progress.
  • Document treatment minutes and regulatory information in the documentation system.
  • Participate in required trainings and adhere to company policies and code of conduct.
  • Collaborate with supervisor and health team members regarding patient progress and plans.
  • Provide counseling to patients and families related to physical disorders.

Principal Responsibilities
  • Consult with patients to learn about their physical condition and symptoms. Review their medical history and referrals from doctors or surgeons.
  • Diagnose movement dysfunction by listening to patients and observing them as they move.
  • Develop individualized treatment plan
  • Coach patient and teach them therapeutic massage to ease patients’ pain and provide stimulation to promote healing
  • Maintain patient record by keeping track of goals
  • Utilize standardized assessments and evidence-based practice to support clinical interventions.
  • Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
  • Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
  • Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
  • Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
  • Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
  • Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy. Accurately records treatment minutes for services provided per all regulatory and clinical requirements.
  • Participate in facility and company required trainings, in-services and conferences.
  • Able to supervise and support physical therapist assistants; rehabilitation aides and/or students.
  • Adherence to Company required policy and procedures and code of conduct.
  • Active State Licensure
