Job summary

Sweetwater Care in Dillon, MT is seeking a licensed Physical Therapist to evaluate and treat patients with movement dysfunction. You will consult on conditions, develop individualized plans, and coach patients in therapeutic techniques.

You will supervise PT assistants and rehabilitation staff, maintain accurate records, apply evidence-based methods, and provide discharge summaries. Collaboration with the health team and ongoing training are essential to ensure compliant, quality care.

Responsibilities

• Consult with patients to learn about their physical condition and symptoms and review medical history.
• Diagnose movement dysfunction by listening to patients and observing them as they move.
• Develop individualized treatment plans.
• Coach patients and teach therapeutic massage to ease pain and promote healing.
• Maintain patient records and track goals.
• Utilize standardized assessments and evidence-based practice to support interventions.
• Assess progress and adjust treatments per regulatory and clinical requirements.
• Evaluate treatment outcomes and modify services; refer as needed.
• Provide consultation to patients, families, caregivers and other providers.
• Communicate progress and plans with supervisor and health team members.
• Provide discharge summaries of services per regulatory and clinical requirements.
• Document all required information in the electronic system and maintain records.
• Participate in trainings, in-services and conferences.
• Able to supervise and support PT assistants, rehab aides and students.
• Adhere to company policies and code of conduct.
• Maintain active State Licensure.

Principal Responsibilities

• Consult with patients to learn about their physical condition and symptoms. Review their medical history and referrals from doctors or surgeons.
• Diagnose movement dysfunction by listening to patients and observing them as they move.
• Develop individualized treatment plan
• Coach patient and teach them therapeutic massage to ease patients’ pain and provide stimulation to promote healing
• Maintain patient record by keeping track of goals
• Utilize standardized assessments and evidence-based practice to support clinical interventions.
• Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
• Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
• Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
• Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy. Accurately records treatment minutes for services provided per all regulatory and clinical requirements.
• Participate in facility and company required trainings, in-services and conferences.
• Able to supervise and support physical therapist assistants; rehabilitation aides and/or students.
• Adherence to Company required policy and procedures and code of conduct.
• Active State Licensure
