Job summary

HealthForce Healthcare is seeking a Physical Therapist for a travel assignment in a Skilled Nursing Facility in Albuquerque, New Mexico. The position runs for 13 weeks starting December 1, 2026, with a 35-hour workweek and a 5x8 daytime schedule.

The role focuses on comprehensive evaluations, individualized treatment plans, gait and transfer training, and discharge planning in collaboration with an interdisciplinary team.

Qualifications

• Active physical therapy license to practice in the United States.
• Ability to work effectively in a skilled nursing facility environment.
• Strong assessment, documentation and communication skills.
• Ability to collaborate within an interdisciplinary team.
• Availability for a 13-week assignment with guaranteed hours.

Responsibilities

• Perform comprehensive PT evaluations for residents in a skilled nursing facility.
• Develop and implement individualized treatment plans aimed at improving mobility and functional independence.
• Provide therapy programs including therapeutic exercise, gait training, balance activities, transfers, and activities of daily living training.
• Reassess resident progress regularly and update documentation and care plans accordingly.
• Collaborate with an interdisciplinary team (nursing, OT, SLP) and communicate with residents and families to support discharge planning and goals.
• Ensure compliance with professional standards and facility policies; maintain accurate records and timely charting.
