Qualifications

• Active Physical Therapist (PT) license required.
• BLS Certification required.
• SNF or rehabilitation experience preferred.
• Experience with geriatric patient populations preferred.
• Strong communication and documentation skills required.
• Ability to work independently and collaboratively within a rehab team.

Responsibilities

• Evaluate and treat patients in SNF, developing rehab plans to restore independence.
• Document patient progress per clinical and regulatory standards.
• Collaborate with interdisciplinary teams to implement care plans.
