Responsibilities

• Perform initial evaluations and ongoing reassessments to identify functional goals and develop a treatment plan.
• Implement gait, balance, strength, ROM, functional mobility and ADL interventions.
• Document sessions in the medical record and communicate progress to the team.
• Monitor safety, apply modalities, and adapt plans based on patient response and goals.
• Educate residents and families on home exercise programs and fall prevention.

Responsibilities

• Perform initial evaluations and ongoing reassessments of residents’ physical abilities to identify functional goals and develop a individualized treatment plan.
• Implement evidence-based PT interventions focusing on gait, balance, strength, ROM, functional mobility, and activities of daily living.
• Document treatment sessions clearly in the resident’s medical record and communicate progress to the interdisciplinary team.
• Monitor safety, apply appropriate modalities and therapeutic exercises, and adapt plans based on patient response and goals.
• Educate residents and families on home exercise programs, fall prevention, and transfer/ambulation strategies to support continuity of care after discharge.

Requirements

• Active Physical Therapist license in New Mexico or eligibility to practice in New Mexico
• Ability to work in a skilled nursing facility setting with geriatric rehab needs
• Availability to commit to a 14-week travel assignment with 35 guaranteed hours per week
• Strong clinical assessment, documentation, and communication skills for collaboration with an interdisciplinary team
• Prior SNF or geriatric rehabilitation experience preferred
