Centra Healthcare Allied is seeking a travel Physical Therapist for a contract assignment in Placerville, California. The role involves providing skilled PT services in SNF/LTC settings, developing a plan of care, and collaborating with the care team to meet patient goals.

The position is 13 weeks, 40 hours per week, with start date 12/05/2026. Current state PT licensure and CPR certification are required, as well as graduation from an accredited program.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist/PT or in process.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services/interventions per physician orders.
• Assess patient needs, plan, evaluate and modify care to meet goals.
• Collaborate with all disciplines to plan and evaluate team goals.

Responsibilities

• Provide skilled physical therapy services in accordance with physician orders.
• Develop and execute individual care plans for PT patients.
• Collaborate with the care team to achieve patient goals.
