This role focuses on delivering quality PT services under physician orders, developing care plans, and collaborating with the care team. Responsibilities include assessing patient needs, providing skilled interventions, and ensuring care modifications align with goals.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist (PT) or actively pursuing licensure.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions per physician orders.
• Assess patient needs, plan, evaluate and modify care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
