You will evaluate patients, develop treatment plans, and collaborate with local teams to ensure mobility and independence. A PT degree and active license, plus travel willingness and a valid license are required.

Qualifications

• Must have a degree from an accredited Physical Therapy program.
• Active PT license or ability to obtain licensure in assigned states.
• At least one year of clinical experience.
• Willingness to travel 100% to assigned locations.
• Valid driver's license and strong communication skills.

Responsibilities

• Travel to assigned Elara Caring locations to provide home health physical therapy.
• Evaluate patients, develop treatment plans, and help them regain mobility.
• Collaborate with local clinical teams to deliver patient care.
• Document patient progress and support continuity of care.
