Affinity Home Health Care Inc. is seeking a dedicated Outpatient Physical Therapist (PT OP) to evaluate patients, develop and adjust individualized treatment plans, and deliver therapy services in an outpatient setting in Colorado Springs.

Responsibilities include comprehensive evaluations, documenting notes, ensuring care aligns with physician orders and insurance requirements, educating patients and families, and participating in professional development and organizational initiatives.

Qualifications

• Active state physical therapy license
• CPR certification
• Degree from an accredited school in physical therapy
• 6 months to 1+ year outpatient experience preferred
• Ability to float between departments if required

Role

Qualifications & Skills

Active physical therapy license for the state, CPR certification, degree from an accredited school, with 6 months to 1+ year outpatient experience preferred. Must be able to float between departments if required.

Additional Requirements

Maintain state licensure, adhere to practice acts, participate in performance improvement, and operate in an 8‑hour day shift, minimum 40 hours weekly.
