Conduct patient evaluations, develop individualized treatment plans, collaborate with interdisciplinary teams, document patient progress using Epic, and educate patients and families.
Minimum of 1 year of experience as a Physical Therapist, current AK license, BLS certification, with preferred experience in pediatrics and outpatient care.
Provide therapy services for patients of all ages and diagnoses, focusing on wound care, lymphedema, pelvic floor, and pediatrics. Maintain accurate records, support patient education, and work effectively within healthcare teams.
