Job summary

Centra Healthcare Allied is seeking a travel Physical Therapist for a travel job in Lancaster, Ohio. LTAC setting, 40 hours per week, 13 weeks, start 10/12/2026. Duties include developing care plans and providing skilled PT services per physician orders.

Requirements include graduation from an accredited program, active or pending PT licensure, CPR certification, and collaboration with multidisciplinary teams to meet patient goals.

Qualifications

• Must graduate from an accredited school.
• Current state license as a Registered Physical Therapist / PT or actively pursuing licensure.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions per physician orders.
• Assess patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
