Job summary

Healthforce is seeking a Physical Therapist for a Skilled Nursing Facility travel assignment in Pittsfield, Massachusetts. Start date is 09/07/2026 for 13 weeks, with a 40-hour week and shift from 08:00 to 16:30. This temporary role supports SNF rehab in a skilled nursing setting.

The ideal candidate holds an MA license with eligibility to practice in the United States and is comfortable working in a SNF environment with a 40-hour weekly schedule.

Qualifications

• Active Physical Therapist license or eligibility to practice in Massachusetts.
• Ability to work in a Skilled Nursing Facility setting and manage a patient caseload.
• Strong documentation, communication, and collaboration skills.
• Eligible to work in the United States.
• Willingness to work a 40-hour weekly schedule.

Responsibilities

• Assess residents’ physical abilities and develop plans to improve mobility, strength, balance, and daily function.
• Implement therapy interventions, including gait training, transfers, endurance activities, and pain management.
• Reassess progress, document outcomes, and adjust plans of care with the interdisciplinary team.
• Educate residents and families on home programs, safety, activity modification, and self-management.
• Ensure accurate documentation, timely progress notes, and regulatory compliance.
• Promote safety during therapy sessions and coordinate with the care team to optimize outcomes.
