Qualifications

• Active Physical Therapy License in the state of employment (MA)
• Current BLS/CPR Certification
• Current TB test/results
• Current fitness to duty statement (within past year)

Responsibilities

• Deliver physical therapy treatments to patients in SNF
• Assess progress and adjust treatment plans
• Collaborate with interdisciplinary team for optimal outcomes
• Educate patients and families on plans and goals
• Maintain accurate documentation
