Job summary

Fusion Medical Staffing-Therapy is seeking a travel Physical Therapist for a 13-week assignment in Lafayette, IN. The role offers 40 hours per week, days shift, and the opportunity to gain diverse clinical experience in hospital, clinic, or rehab settings.

Candidates must hold a valid PT license in Indiana and current BLS certification; Doctorate is preferred. Travel pay, comprehensive benefits, and support from a dedicated clinical team are included.

Qualifications

• Valid Physical Therapy license in Indiana.
• Current BLS certification (AHA/ARC).

Responsibilities

• Evaluate patients’ physical conditions and identify functional limitations.
• Develop personalized rehabilitation programs based on assessments and goals.
• Implement treatment plans to improve mobility and reduce pain.
• Document evaluations, interventions, and outcomes accurately in medical records.
• Collaborate with physicians, nurses and therapists to coordinate care.
• Educate patients on home exercise programs and injury prevention.
• Ensure a safe environment and adhere to infection control and regulatory standards.
• Contribute to discharge planning and community/home-based transitions.
