Job summary

Cross Country Allied is seeking a travel Physical Therapist for a travel job in Chippewa Falls, Wisconsin. The PT will provide home health services to geriatric patients, handling approximately 57 visits per day, with 8-hour day shifts and a 40-hour work week.

The position requires 1 year of relevant experience, a current PT license, BLS, and an active driver’s license. On-call rotation and solid communication are expected.

Qualifications

• Minimum of 1 year of recent relevant experience.
• BLS certification required.
• Physical Therapist license in hand at submission.
• Active driver’s license and reliable transportation.
• OASIS experience required for SOC visits.
• HCHB experience required.
• Strong documentation and communication skills.

Responsibilities

• Provide home health Physical Therapy services to geriatric patients.
• Manage approximately 57 patient visits per day.
• Timely documentation and inter-team communication.
• Participate in on-call rotation.
