Job summary

Jackson Therapy Partners is seeking a traveling Physical Therapist for a home health assignment in Marquette, Michigan. The position entails 40 hours per week, Monday through Friday, with shifts from 8:00 AM to 4:30 PM, serving Marquette County and nearby areas.

Mileage is reimbursed, and a MI license plus active state credentials are required. Responsibilities include admissions, evaluations, progress notes, discharges, and collaboration with PT/ PTA teams.

Qualifications

• Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA.
• Active State License is Required to Start the Assignment.
• BLS Certification may be Required from AHA or ARC.
• Home Health Experience preferred.
• Physical Therapist License in MI.

Responsibilities

• Admissions, add-on evaluations, reassessments and discharges.
• Create individualized plans of care and progress notes.
• Supervise and collaborate with PTA and support staff.
• Participate in weekly case conferences and monthly staff meetings.

Assignment Details

• Shift Hours: 8:00 AM - 4:30 PM.
• Shift Days: Monday through Friday.
• No on-call requirement.
• Coverage area includes Marquette County and the western portion of Alger County within a 1-hour radius of Marquette.
• Mileage is reimbursed between visits, from first patient to last patient.
• Responsibilities include physical therapy admissions, add-on evaluations, reassessments, discharges, creating individualized plans of care, supervising and working in conjunction with PTA, and participating in weekly case conferences and monthly staff meetings.
