Job summary

American Traveler is seeking a Physical Therapist for a Home Health position requiring at least 1 year of home health experience and state licensure.

Responsibilities include skilled visits, patient assessments, treatment planning, and documentation. Travel between patient homes with provided laptop and equipment; 5x8 schedule, some weekends, and vaccination policies apply.

Qualifications

• Active state PT license required if applicable
• BLS certification required
• Minimum 1 year of home health experience required
• OASIS training preferred
• Must have a valid driver''s license and reliable vehicle for home visits
• Experience with adaptive devices, gait belts, walkers, slide boards, collars, and wheelchair assessments required
• Skilled in assessment, documentation, orthopedic and neurologic care, and cardiopulmonary/circulatory conditions
• Knowledge of home exercise programs, medication management, and patient/family education required
• Must be able to recognize signs of physical, sexual, or psychological abuse
• Billing knowledge across commercial, Medicare A, private pay, and worker''s compensation required

Responsibilities

• Home Health setting providing skilled visits to patients under Medicare, Medicare Advantage, and commercial insurance
• Patient population includes med/surg, cardiac, ortho (hips/knees/shoulders), post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson''s, failure to thrive, respiratory, neuro, trachs, trauma, AKA/BKA, and ADL-related conditions
• Day shift schedule, 5x8 hours (8:00 AM - 5:00 PM)
• 6-8 home skilled visits per day (minimum of 6 points per day, may increase by 1 at manager''s discretion)
• 1-2 weekends required per contract
• Uses BayCare Forms, BayCare Scheduling, and Ace Timesheet documentation systems
• Provided with laptop, MiFi for internet access, and portable printer
• Responsibilities include patient assessment, treatment planning, home exercise programs, and documentation of care
• Candidate must comply with COVID-19 vaccination or exemption policy
• Flu vaccination or declination required, with mandatory mask use at work if declined
• Any solid color scrub accepted as uniform
• Local candidates accepted at the same pay rate
• Open to first-time travelers
