Qualifications

• 1 professional reference from entire work history required.
• COVID vaccination card or approved declination required.

Responsibilities

• Home health setting covering south Lake County.
• Ortho case load with 4-6 visits per day.
• Schedule is Monday through Friday, 8:30am-5pm.
• 1 weekend rotation per month required for ortho case visits.
• Coverage required for 1 major and 1 minor holiday.
• No on-call requirement.
• Weekend or holiday call-offs may result in rescheduling to another weekend or holiday per facility policy.
• Mileage varies by week based on caseload.
• Local candidates accepted at the same rate as travelers.
• No radius rule in place.
• Former permanent employees must have a 90-day break in service before returning as a traveler.
