Job summary

Ardor Health Solutions is seeking a travel Physical Therapist for a 13-week contract in Chesapeake, VA, at a skilled nursing facility. The role focuses on geriatric rehabilitation, gait training, balance, and functional independence, with a 12-hour shift and 35 hours per week.

The successful candidate will hold an active Virginia PT license, preferably SNF-experienced, and can work independently while collaborating with an interdisciplinary team. New travelers are welcome to apply.

• Strong communication and documentation skills.
• Ability to work independently and manage a patient caseload.

Responsibilities

• Develop individualized treatment plans for each patient.
• Provide gait training, transfers, balance, mobility, and functional independence.
• Collaborate with nursing, therapy, and other healthcare professionals as part of an interdisciplinary team.
• Document patient progress and maintain accurate records throughout the plan of care.
