Qualifications

• Active PT license in the United States or eligibility as required by state regulations.

Responsibilities

• Assess patients and develop, implement, and adjust rehabilitative plans to improve mobility, relieve pain, increase strength, and prevent deformity.
• Deliver PT interventions across the full continuum of care, using evidence-based practices to advance function and independence.
• Document evaluations, treatment plans, progress notes, and outcomes in a timely, compliant manner.
• Collaborate with physicians, nurses, and other care team members to optimize patient outcomes and support discharge planning.
• Ensure safety and regulatory compliance in therapy delivery, with adherence to all applicable standards and policies.
• Educate patients and families on home programs, safety, and strategies to maintain gains after discharge.
