Responsibilities

• Conduct comprehensive PT evaluations for home health patients, including functional assessments and development of an individualized plan of care with measurable goals
• Deliver and adjust evidence-based physical therapy interventions (gait training, balance, strengthening, mobility) within the patient’s home, prioritizing safety and fall prevention
• Reassess patient progress during visits, monitor outcomes, and modify goals and treatment plans; document progress in EPIC per agency standards
• Complete Start of Care and subsequent revisits, including timely data collection and OASIS submission as required
• Collaborate with an interdisciplinary team (nursing, therapy, social work) to coordinate care and plan for safe discharge and community resource referrals
• Educate patients and caregivers on home exercise programs, activity modification, safety practices, equipment use, and adherence to the plan of care

Requirements

• Active Physical Therapist license in Pennsylvania
• Recent home health experience
• EPIC EMR experience

• Ability to work in a home health setting and commit to a 12-week travel assignment
