Job summary

AccessHealth in Rutland, Vermont seeks a Long Term Care Physical Therapist for a travel-based role in home health and hospice settings. You will perform comprehensive OASIS and PT assessments, develop care plans, and deliver skilled therapies in patients\' homes, coordinating with an interdisciplinary team.

You\'ll supervise PTAs and students, ensure regulatory compliance, and discharge patients safely, with weekday hours and occasional weekend shifts. Travel to patients\' homes is required.

Qualifications

• Minimum of 2 years of physical therapy experience.
• Proficiency in OASIS assessments.
• Current state licensure as a Physical Therapist.
• Knowledge of APTA guidelines and home health regulations.
• Ability to perform wound care and blood pressure monitoring.
