Job summary

Personal Touch Home Health Care Services Inc. is seeking a Travel Home Health Physical Therapist to provide patient-focused therapy across a designated coverage area.

You will assess, treat, and document progress during scheduled shifts, ensuring quality care while complying with licensure requirements. The role requires a Bachelor’s/Master’s/Doctoral degree in Physical Therapy, an active state license, reliable vehicle, and good communication.

Qualifications

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited institution
• Active state license for Physical Therapy
• BLS certification if required
• Must have a reliable vehicle, valid driver's license, and insurance
• Strong communication and time management skills are essential

Responsibilities

• Deliver therapy services Monday through Friday, 8:00 AM to 4:30 PM
• Cover locations within fifty miles of the assigned area
• Maintain documentation and ensure patient progress

Education

Physical Therapy degree (Bachelor's/Master's/Doctoral)

Active state license

Assist patients in recovery from injuries and manage chronic conditions, providing therapy services within a designated coverage area. Ensure quality patient care during scheduled shifts, and adhere to licensing and certification requirements. Mileage reimbursement is provided for travel between visits.

Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited institution, active state license, and BLS certification if required. Must have a reliable vehicle, valid driver's license, and insurance. Strong communication and time management skills are essential.
