Job description

Role: Travel Home Health Physical Therapist responsible for patient recovery and chronic condition management in various locations.

• Responsibilities: Provide physical therapy services, assess patient needs, develop treatment plans, and ensure quality care. Manage a caseload across multiple sites, adhere to productivity and dress code standards, and assist with coverage area needs. Mileage reimbursement and flexible travel options offered.
• Qualifications: Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program, active state license, and professional PT certification. Home health experience and BLS certification may be required.
• Skills: Strong clinical assessment, communication, and organizational abilities. Ability to work independently in diverse settings while maintaining high-quality patient care.
  

Key Duties:

Deliver therapy services Monday through Friday, 8:00 AM to 4:30 PM, with additional on-call coverage. Travel within designated areas, meet productivity expectations, and comply with dress code. Reimbursements for mileage, travel, and licensing support included.
