Job summary

Personal Touch Home Health Care Services Inc. is seeking a Travel Skilled Nursing Facility (SNF) Physical Therapist to support multiple facilities across Indiana. You will assess patients, deliver therapy, and coordinate care with clinical teams in a fast-paced, travel-friendly role.

The ideal candidate holds a PT degree (DPT preferred), active state license, and BLS certification. Prior SNF experience is preferred.

Qualifications

• PT degree and state licensure in good standing (DPT preferred).
• BLS certification may be required.
• Prior SNF experience is preferred.
