Thrive Staffing seeks a dedicated Travel Physical Therapist for a 13-week assignment in Ukiah, California. The role requires full-time, 40 hours weekly, with options for 8-hour day shifts and flexible start times (7:30 AM or 8:00 AM).

Expect a 30–60 minute lunch and travel-friendly scheduling. You will conduct assessments, develop treatment plans, provide therapy, monitor progress, and document in EPIC, collaborating with the care team to ensure optimal patient outcomes.

Qualifications

• Must hold a DPT degree or foreign equivalent.
• Valid California PT license is required.
• CPR/BLS certification is required per policy.
• Experience with EPIC EMR is preferred.

Responsibilities

• Perform patient assessments and develop individualized treatment plans.
• Deliver therapeutic activities and exercise programs.
• Monitor progress and adjust plans as needed.
• Document patient records in EPIC accurately.
• Collaborate with the healthcare team to ensure comprehensive care.
• Educate patients and families on exercises and health maintenance.
