Centra Healthcare Allied is seeking a travel Physical Therapist for a travel job in Clearwater, Florida. This SNF/LTC role requires a dedicated clinician for a 13-week assignment with 8-hour day shifts.

Qualified candidates have graduated from an accredited program and hold a current state PT license or are in licensure process, and will develop care plans, deliver skilled PT services, and collaborate with the care team to meet patient goals.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist or in process of licensure.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services/interventions per physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.

This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements include:

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or proactively in the process of the application process for current state licensure as a Registered Physical Therapist / PT.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions in accordance with physician orders.
