Qualifications

• Licensed physical therapist in the state
• Current BLS certification (AHA/ARC)
• Doctorate degree preferred (PT)

Responsibilities

• Evaluate patients’ physical conditions and identify treatment needs
• Develop and implement personalized rehabilitation programs
• Document evaluations, interventions, and outcomes accurately
• Collaborate with physicians, nurses, and other professionals to coordinate care
• Educate patients and caregivers on home exercise programs and safety
• Maintain a safe treatment environment and infection control practices
• Support discharge planning and transitions to home or community settings
