Responsibilities include delivering PT services in SNF and Home Health settings, supervising two PTAs, and using Net Health and HCHB for documentation. New grads welcome; valid driver's license required.

• Day shifts, 8 hours, 40 hours per week, Sunday through Saturday.

Responsibilities

• Care setting includes SNF and Home Health, split approximately 50/50, with additional coverage assisting Auburn and Syracuse locations based on PTO and census
• Day shift schedule, 8-hour shifts, 40 hours per week
• Work week runs Sunday through Saturday
• Supervises 2 PTAs
• Onsite Therapy Supervisor available
• Uses Net Health for SNF documentation and HCHB for Home Health documentation
• Productivity expectations include 80% SNF productivity and a points-based system (approximately 80 minutes per patient, or 27 points if full-time in Home Health)
• No call or call-back requirements
• Responsibilities include providing physical therapy services across SNF and Home Health settings and supervising PTAs
• Accepts first-time travelers
• Local candidates accepted at an adjusted rate
• 80-mile radius rule applies for local candidate determination
