Job summary

Work includes delivering outpatient rehabilitation services in a clinical setting, collaborating with an interdisciplinary team, and guiding patients through treatment plans and home exercise programs.

Qualifications

• Current physical therapy license to practice in California or eligibility for licensure.
• Experience in outpatient rehabilitation or strong interest in outpatient PT practice.
• Ability to participate effectively in an interdisciplinary team and collaborative care model.
• Availability to start on 08/31/2026 and commit to a 13‑week travel assignment.
• Willingness to work a 40‑hour week on a 4x10 schedule and travel as required.

Responsibilities

• Plan and conduct segments of outpatient treatment programs designed to restore and improve physical function, and support social and mental well‑being within program objectives.
• Assess patient groups and monitor progress, adjusting therapy plans to meet individualized goals.
• Co‑lead interdisciplinary care activities, collaborating with other professionals to coordinate patient care.
• Educate patients and caregivers on treatment strategies, home exercise programs, and safety planning.
• Document patient status, goals, and outcomes in accordance with department policies and regulatory requirements.
• Ensure adherence to approved treatment protocols and maintain patient-centered, evidence‑based care.
