Job summary

HealthForce Healthcare is seeking a Physical Therapist for an outpatient assignment in Fort Bragg, CA. The 13-week travel position requires working a 4x10 schedule (40 hours per week) in a clinical outpatient setting, with ongoing support from HealthForce Healthcare for onboarding, credentialing, and travel arrangements.

Candidates must hold a California PT license (or be eligible) and have experience or strong interest in outpatient rehabilitation.

Qualifications

Current physical therapy license to practice in California or eligibility for licensure.
Experience in outpatient rehabilitation or a strong interest in outpatient PT practice.
Ability to participate effectively in an interdisciplinary team and collaborative care model.

Responsibilities

Plan and conduct segments of outpatient treatment programs designed to restore and improve physical function, and support social and mental well-being within program objectives.
Assess patient groups and monitor progress, adjusting therapy plans to meet individualized goals.
Co-lead interdisciplinary care activities, collaborating with other professionals to coordinate patient care.
Educate patients and caregivers on treatment strategies, home exercise programs, and safety planning.
Document patient status, goals, and outcomes in accordance with department policies and regulatory requirements.
Ensure adherence to approved treatment protocols and maintain patient-centered, evidence-based care.
