Job summary

Healthforce seeks an Occupational Therapist (General, travel assignment) for a 13-week posting in Newport, Vermont. Start date is 10/26/2026 with weekly pay between $2,444 and $2,638 and guaranteed 36 hours per week in a Skilled Nursing Facility.

You will perform comprehensive assessments, develop individualized plans to improve self-care and mobility, document progress, and collaborate with the interdisciplinary team. Vermont license and OT degree are required, with strong communication skills.

Qualifications

• Active VT OT license or eligibility for VT licensure.
• Degree in Occupational Therapy from an accredited program (Master’s or Doctorate).
• Ability to work effectively in a skilled nursing facility setting.
• Strong assessment, documentation, and treatment-planning skills.
• Excellent communication and teamwork abilities.

Responsibilities

• Conduct comprehensive occupational therapy assessments for residents in a skilled nursing setting.
• Develop and implement individualized treatment plans aimed at improving self-care, mobility, and functional independence.
• Deliver evidence-based OT interventions, including activities and modalities tailored to residents’ goals.
• Document progress, adjust treatment plans as needed, and communicate findings to the interdisciplinary team.
• Educate residents and families on therapy goals, home exercise programs, and safety strategies.
• Collaborate with nursing, therapy, and social work staff to support discharge planning and transitions of care.
