Job Description

We are seeking a dedicated Occupational Therapist to provide in-home therapy services aimed at maximizing the independence and safety of our home care and hospice patients. The successful candidate will work both independently and as a collaborative member of our multidisciplinary team.

Responsibilities

• Deliver in-home therapy services to patients.
• Develop treatment plans to enhance patient independence and safety.
• Collaborate with a multidisciplinary team to provide comprehensive patient care.
• Maintain accurate documentation of patient assessments and progress.
• Provide education and support to patients and their families regarding occupational therapy techniques and goals.

Required Experience / Certifications / Licensure

• Current Vermont licensure as an Occupational Therapist.
• Valid driver’s license and reliable vehicle.
• Experience in a home care setting is preferred.

Assignment Duration

This is a 13-week contract position.
