Responsibilities

• Conduct initial occupational therapy evaluations for residents in the skilled nursing facility, assessing mobility, ADLs, balance, cognition, and safety.
• Develop and implement individualized treatment plans; provide direct therapy and adjust plans based on progress; document outcomes.
• Deliver therapy to improve independence, endurance, strength, coordination, and cognitive skills; educate on home exercise programs and safety.
• Collaborate with the interdisciplinary team; participate in rounds; support discharge planning and transitions.
• Supervise therapy assistants or support staff; ensure adherence to scope of practice and facility policies.
• Maintain compliant, timely documentation in the electronic health record and ensure HIPAA compliance.
