Responsibilities

• Conduct initial patient evaluations to determine PT needs for residents in a skilled nursing setting.
• Develop and implement individualized treatment plans with measurable, function-focused goals.
• Provide evidence-based interventions (e.g., therapeutic exercise, gait and balance training, functional mobility, manual therapy as appropriate).
• Reassess progress, document outcomes, and adjust plans to maximize functional recovery.
• Collaborate with an interdisciplinary team (nursing, occupational therapy, speech therapy, physicians) to coordinate care and progress toward discharge.
• Educate patients and families on home exercise programs, safety, and strategies to maintain gains after discharge.

Requirements

• Active physical therapist license in California
