Responsibilities

• Screen/evaluate patients in short-term and long-term rehab settings and potential outpatient
• Use standardized assessments and evidence-based practice for interventions
• Provide rehabilitative, skilled, and medically necessary treatment per plan of care
• Administer a range of interventions based on individual patient needs
• Assess progress and adjust treatments in the clinical record per requirements
• Provide consultation to patients, families, and other providers regarding physical disorders
• Communicate with the OneClinical Team about progress and barriers
• Provide comprehensive discharge summaries as required
