Job Description

Your primary responsibility will be to provide in-home health care as a Physical Therapist, Occupational Therapist, or Speech-Language Pathologist. You will deliver evaluation, treatment plans, and interventions, ensuring quality care for patients.

Responsibilities

• Conduct evaluations and develop individualized treatment plans.
• Implement therapeutic interventions and monitor patient progress.
• Document patient care in Homecare Homebase (HCHB) accurately.
• Participate in weekly case conferences.
• May cover Shasta Lake south to Orland, and assist in other areas as needed within a 60-mile radius.

Required Experience / Certifications / Licensure

• Experience in Home Health and OASIS documentation is required.

• Valid state licensure as a Physical Therapist, Occupational Therapist, or Speech-Language Pathologist.
