Licensure:

• LARA and/or Nursys must state Active Michigan/MI License [REQUIRED]

Additional Requirements:

• Must pass a dysrhythmia test prior to working on the floor
• Traveler must have their own vehicle for this need
• Covid Vaccine Required, exemptions allowed

Documentation:

• Skills Checklist, current to 1 year, pertinent to the specialty (OB/GYN) [REQUIRED]
• 2 Supervisory Reference Forms Completed within the last two years [REQUIRED]
