Job Summary

We seek a friendly and compassionate Travel Nurse Practitioner to support patient onboarding into the Patient Health Management (PHM) program. You will work with care coordinators across various providers and settings to promote better health outcomes, facilitate patient assessments, communicate between facilities and patients, and ensure accurate documentation using our proprietary software. This role offers an opportunity to make a meaningful impact on patient health in a dynamic team environment.

Responsibilities

• Onboard patients into the PHM program via telehealth, including benefits and informed consent.
• Assist care coordinators across different providers and settings.
• Enhance health outcomes through effective communication between facilities and patients.
• Assess patient conditions and report through our proprietary software system.
• Cross-train in other areas of care management.
• Gather and analyze patient information.
• Review patient labs, studies, and provider notes thoroughly.

Requirements

• Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license.
• State-specific license in good standing in New Jersey.
• Minimum 6 months of experience in long-term care or skilled nursing facilities.
• Strong communication, clinical, and critical thinking skills.
• Passion for elder care and person-centered healthcare.
• Reliable transportation and willingness to travel up to 100% of the time.
