Position: Travel Nurse Practitioner

Monogram Health is seeking a skilled Nurse Practitioner eager to make a difference in patients' lives. The Travel Nurse Practitioner plays a vital role in providing comprehensive care to underserved patients within our Monogram Health network. The goal is to improve patients' quality of life at home, slow kidney disease progression, and manage other chronic illnesses, leading to positive health outcomes.

Your Impact:

As a Travel Nurse Practitioner, you will deliver personalized, compassionate care to patients primarily with CKD through ESRD. You will serve as a nationwide float provider, reaching areas where local clinicians are unavailable or where our services are expanding. Your duties include patient health assessments, developing management strategies, promoting health habits, maintaining accurate records, scheduling follow-ups, and collaborating with healthcare teams.

Roles and Responsibilities:
  • Travel nationwide for in-home assessments, wellness exams, and screenings, both in-person and virtual
  • Travel up to 75% of the month during work weeks
  • Educate patients and families on health benefits and programs
  • Document clinical information accurately and timely
  • Coordinate care with primary care providers, specialists, and ancillary services
  • Ensure quality care standards are met
  • Work remotely based in your state, attending periodic training outside your state
  • Prescribe medications, order tests, and collaborate with physicians
  • Participate in care team meetings and educational initiatives
Position Requirements:
  • Active, unrestricted RN and Nurse Practitioner licenses
  • Board certification (ANCC/AANP)
  • Current DEA certificate or eligibility
  • Ability to travel extensively
  • Work independently without supervision
  • Valid driver’s license and transportation
  • Proficiency with medical instruments, equipment, and electronic health records
  • Effective communication skills
  • Knowledge of federal and state regulations including HIPAA and OSHA
  • Minimum of 2 years of direct patient care experience
