Job summary

GuideStar Eldercare is seeking a Nurse Practitioner to address dementia and neurocognitive disorders in nursing homes and assisted living facilities. You will manage an assigned caseload with support from corporate clinical leadership and in-house scheduling, coding, and billing.

You will travel daily to facilities, perform initial evaluations, ongoing therapy, and cognitive testing, documenting needs and outcomes while fostering strong relationships with facilities and families.

Qualifications

• Proof of Nurse Practitioner licensure
• Ability to work as part of an interdisciplinary treatment team
• Good time management skills
• Able to function autonomously with regional Area Manager support
• Pass pre-employment background check and drug screening
• Complete credentialing process

Responsibilities

• Travel to assigned facilities from home on a daily basis
• Provide initial evaluations, continuing therapy services, and cognitive testing
• Document the need and extent of services for each patient
• Create and maintain positive professional relationships with client facilities, patients, and their families
